Actions Tab Missing
I am not getting an action tab in prepare form when I select properties in a field in Acrobat DC. I only see tabs for "General", "Appearance", "Position" and "Options" .

It sounds like you (perhaps inadvertently) set up the document to be an Adobe Sign compatible form, which happens when you select the "This document requires signatures" check box when you select Tools > Prepare form initially. The fix is to go into form editing mode and select "More > Revert to Acrobat Form" in the panel on the right, and any fields you add will have the Actions tab available.
