Add number of instances

So I have a form that I want to count the number of times a value has been checked with a value greater then 1.  For example:

So I have a calculation to add the values for total number of SEVERITY.  Create values for the checkboxes.  Now I need a formula or script for the number of symptoms that above a 0 for Vestibular Symptom NUMBER.

Any thoughts??

I also have another list off 22 that I will need to create something similar.
