Adobe Acrobat DC identity auto-changes

Just upgraded my team to Adobe Acrobat DC. Now when performing reviews our Identity changes. Person-A will opens PDF-1 for review and exits. Person-B will open it and comment on it. Person-A now comments on the PDF but notices their Acrobat Identity has changed to be Person-B. This is happening to every person on the team in that their identities are getting mixed up an they don't know who made what comments anymore.  Anyone have a clue as to how or why this happens and how to fix it? Some use Windows 7 others Windows 10.

Sorry for the delay in response.

Could you please confirm the dot version of Adobe Acrobat DC you are using.

Is this happens on all the PDF that you use for shared review?

Have you tried updating application? Go to Help menu > Check for updates.
