Adobe Check Box Field - Another Value Allowed?

Hello,

I am trying to create a PDF form that has several check boxes. However, if the certain field is not applicable, I don't want to "check" the check box, nor do I want to leave it blank. I would like to have a way to make the box say "N/A" for not applicable.

Does anyone know a way this can be done?
