Adobe Form Formula Involving Months Past Due

I have created a fillable form with several formulas.  One of the areas I'm having an issue with is coming up with the javascript for "Interest on Tax Due."

I basically need a formula to do the following:

Interest on Tax Due (Multiply the number of months Past Due times the amount on Line 3, and multiply that by .025).
