How do I insert a gutter or margin in an existing pdf to allow me to comb bind a large document?

Please try "Crop Pages" option under Edit menu in Acrobat application.

Go to Edit menu > Crop pages and double click on page. You will get "Set Page Boxes" dialogue box. as shown below in screenshot.

Check if it works for you.
