Can I create a form without a signature and without the signature message when it opens?

I am trying to create a form for the user to enter and save the username and password information with how-to's. I do not want a signature on it and I would like to not have the message about the signature when the user opens the form. Can I do that and, if so, how?

Thank you. I did learn that I got the message because I did not click on "sign" but just selected a field to fill in. That's a default I can live with. Thank you for your response.
