Can I create a PDf Form Template?

I'd like to distribute a PDf form to my sales team that they could fill out after each sale and submit back to me. I would want the form to be a template so each time they open it all the fields are blank. Can that be done?

You need to carefully think how it's going to work. If the form fields are reset each time the form is opened, then it would also happen when you open the form submitted to you... It might be better to include a Reset Form button instead of doing it automatically.
