Can I create a pdf form which can be printed and then scanned, with all the fields and values, I created in the first form?

I created a pdf form which I now would like to print, then give it to someone who fills it with out with a pen. The next step should be to scan the document and get it back on my PC with the structure of the pdf-form. I don't know if this is even possible ?
