Check Boxes

I created a form, with several check boxes; the form works fine for me, however, when others try to fill in the form, some of the boxes seem to be linked (even though they have nothing to do with each other).  The boxes are all named different things, so I know it's not that.  Even my IT department can't figure this out, any advice is greatly appreciated!
