Check boxes won't appear when accessed on an ipad?

I have created a contract for my salesmen to access and fill out with their customers.  Each time they access it on their Ipad Pro's however, the check boxes are not visible. 

What changes do I need to make on the form to make them visible?

What PDF viewer on the iPad are they using?

How were the check boxes created? Did you use InDesign?

I used Adobe acrobat pro and used the "prepare form" section.  On the Ipad I'm using Adobe Fill and Sign.

The Fill & Sign app is meant to be used for "forms" that don't have any actual interactive form fields. Try using Adobe Reader for documents that have interactive form fields.
