Collect data from a PDF form Using Adobe Acrobat Pro DC and upload to url

What I need is collect certain data from a form created in Adobe Acrobat Pro DC. Creating the form is straight forward and getting the data mailed or tracked through the software itself is simple too but that's not what I need. I need that data in form of an FDF or HTML and then fetch it with minimal user interaction. Below is a sample of the form I shall be using:

While adobe provides the option of export to url, but there aren't a lot of tutorials online that show such a process clearly. You can see the option provided in the export below:

If anyone has ever executed something like this, please guide me through it.
