convert form pdf into word

I'm having trouble converting the simplest Word Documents into a form pdf (using checkboxes) then converting the form pdf back into Word. So, the problem is the last step. When I try to convert the form pdf into the Word document the final Word document has lost the formatting and the checkboxes aren't checked as they supposed to be. Any thoughts? Is this a problem in Windows 10? I'm about to do this on my other computer which is Windows 8.

What's your version of Acrobat, and of Office? Also, how exactly are you

converting the Word document (using what command)?

Word 2016, Acrobat DC.

From within Word, I click on the convert to Acrobat icon.

When you convert a PDF file to a Word file the form fields do not remain in tact, as the two types of documents are very different.
