Create button to insert a PDF file into a pdf file.

I am attempting to create a form checklist for completing tasks that require documentation at my workplace. I would like to create a button that will allow any existing PDF to be inserted into a new page after the checklist. Is it possible to script a button that will allow this? And I would like to use different buttons for each check so that multiple separate pdf files can be inserted after the checklist. Even one button that would allow multiple insertions at different times. Ideally, these files would all be recognized PDF pages that can be sorted and deleted as needed.
