Creating fillable fields in Adobe

Hi everyone,

I work for a grant funded program through the state of Illinois.  Each person referred to our program has a file filled with "intake" documents.  These documents all have the persons name, dob, and a unique id # given to them through our system.  We would like to import these forms to Adobe and type in the information rather than using expensive address labels or hand writing all of these forms.  Is it possible to create fillable fields within an adobe file where all like fields could be linked?  For example, I type the persons name once and it auto fills into all of the other forms?

If the text files have the correct structure then you should be able to import them directly into a PDF file with form fields and populate the fields with the values from the data files.

We currently have the free version of adobe.  can this be done on it or would we need to upgrade the version of adobe we have for our office?

The free Reader will READ forms, and you should be able to enter data in fields that have already been created

As far as I know, you need the paid Acrobat to CREATE the fields
