Custom Alert/Dialog Box

Is it possible to have a custom alert or dialog box with three buttons displaying three different letters and depending on which on was selected it would be displayed in a field on the pdf.

There are some examples (and all the info you need, pretty much) under the documentation of the execDialog method of the app object.
