Custom dialog box script

Can someone please make me a script for dialog box I don't know how to do it myself.

Dialog box should have text first( Il input custom text in code) under that it should have input field for numbers(it's not necessary but it would be nice if it have max number that can be input, lets say 7 is max number that can be input) it should have "ok" button when pressed selected number should be writen in 2 text boxes(text1 and text2) hope someone can help or give me some pointers how to start? Thanks
