Drop down box changes appearance when clicked.

I have a pdf that I use at work, and the other day it was working like normal, I go to lunch and come back and the drop boxes are acting up. What I mean is when I click on the drop boxes, it changes appearance to more of a "Windows-style" drop box and I have to click it twice in order to select any options.

It is Adobe Reader 11.0.15 on Windows 7. I've tried uninstalling and re-installing reader, but it still acts the same. I've also tried using an earlier version of the document, on different computers and the issue is the same everywhere. I'm guessing it's some Adobe display setting, but I can't find anything or anyone else having this problem on forums, etc. Any help would be appreciated, it's pretty annoying.
