Fillable form where each letter is entered into separate box

I am trying to create a form where a user enters their name and address with each letter or number being entered into a separate box. I see these types of forms all the time; however, I cannot figure out how to create one.

On the "Options" tab for the fields, select the "Comb of _____ characters" and fill-in the maximum number of characters for the field.

In older versions or Acrobat we created individual fields for each character and some additional JavaScript and information to automatically tab to the next field.
