Form fields scrambling when saving

I've taken an PDF and have over-layed a form on top.  All looks well in Adobe Acrobat Pro, then when I save it and open it in Preview.app the form fields are completely scrambled (mostly vertical, or the text is vertical).  I've checked the form field orientation properties and it is set to "0" degrees, and have read any other way to change or set these so they don't move.  Any advice is appreciated.
