Formatting Excel File To Import Data Into Acrobat Pro DC

So I have my PDF form all set with the fields named correctly. The issue seems to be the format of my Excel file that contains the data. I must be saving it incorrectly and I have tried multiple .CSV and .TXT formats.

Any hints for me?

I am using Excel 16.0 latest.

I actually was able to clean up the file and now I get this and don't know how to import both records, each into its own form.

You can't do that, not directly at least. This kind of operation is called a Mail Merge. It can be performs using some versions of the PDFMaker plugin for Office, or by using a script, like this one I've developed: Custom-made Adobe Scripts: Acrobat -- Mail Merge and Email
