Four character prefix for my plug-in.

I am studying the Acrobat DC SDK Documentation, which states that I should register a four character prefix with Adobe for my plug-in.  How is that done?  Whom should I contact?

Fill out this form: https://pdfs.semanticscholar.org/1686/416939d54bcbe088557f2a771af07c43be92.pdf

(it’s also on the Adobe servers, but I don’t have that URL handy)

Currently, the form URL(https://pdfs.semanticscholar.org/1686/416939d54bcbe088557f2a771af07c43be92.pdf) cannot be found at this time.

What is the changed URL address?

I sent the form.  Will I be notified when my prefix is registered?  How long does that usually take?
