Generate PDF form (fill-able) using SAP B1

I'm trying to use SAP Business One (via Crystal reports) to create a PDF form.  Some of the data I need is in SAP, but this is for a service report and some has to be filled in by the tech on site.  I know I can create a simple PDF using CR, but is there a way to convert it to a PDF form using a template?
