Having an automatic "Question" asked when PDF opens - then link answer to another action

I would like to set up an "action" whereby when a document is opened there is an automatic "pop-up" question (asking "Have you read and sign the Acknowledgement Form?  If 'not' you will be directed here to do so.  If 'yes' please ignore this message")

Anybody have any idea how I could write this up as a New Action or New Custom Command in Acrobat Pro DC?
