Hide/show fields based on radial selection

I have a radial section with 4 options. They're connected so that only 1 option is allowed. I'm looking to have each option tie to fields in the form so that certain fields are hidden or showing if a specific radial option is selected.

 

In this picture, in the Reason area, there are 4 radials. If we select "New Drawing", I am trying to have the lower half of the screen to hide the 2 drop downs listed as well as the fields undert location, quantity, and tag #. If we select "New Revision", I want those hidden fields to show. How can I achieve this?

We can't help you much if you don't share actual field names and radio button choices, along with conditions to show/hide fields.
