How can I create form fields in a pdf form

I am trying to build a fillable form where there are form fields to tab thru when entering customer information and pictures. Then I need it to print out as a report in the same form style with all the information added but not showing the form field box. Need to figure this out as soon as possible.

To answer your question: You can add form fields using Acrobat. If you'll let us know what version you have we can give more exact information, but I suggest you also check out the online help tutorials about form fields. They explain all the basic steps.
