Since you posted in the Acrobat SDK, I assume you're a programmer. Please confirm you have Acrobat Pro (not the free Acrobat Reader), and tell us which API you are working with (e.g. JavaScript, COM, plug-in...)

What if I don't want to insert it from a file?  I just want to put a blank page in between two existing pages.

First of all, you can do that using a blank page file. If you want to insert a blank page use Tools - Pages - More Insert Options - Insert Blank Page. There are also a bunch of free scripts around that allow you to do that with a bit more customization options, such as deciding what size the page should be (the default option in the built-in tool is Letter: 8.25" x 11").
