How do I change the address that I used when creating a fill able form?

How do I change the address that I used when creating a fill able form? ( the email address and name that my recipient will see when they receive the fill able PDF form)? I did something for work on my own mac and adobe account and now if I want to send personal read able forms, I do not want the the original email used as 'sender'
