How do I get a PDF to stop auto-populating fields?

I'm trying to fill out a form for graduate school and enter in the dates for different jobs I've worked. When I add a date for my first job though, all the other date fields populates with the date I entered.  If I try to delete it, it deletes all the dates. I've turned off the auto-fill function but nothing seems to work.
