how do I include a jpeg field into a fillable form

I need help. I have made a form for our pony club to use at different horse shows and rallys. One of the requirements is that a picture of the club members horse must be on the form. I havent been able to figure out how to add a field to add a picture from a file into the fillable form. Help please.

It's fairly easy to convert images to PDF these days. All mac users and most Windows users will have software already installed that can do it, so if you have motivated users and clear instructions, it can work.

What application is being used by the users to fill in the form? If it's Adobe Reader then it can't be done. They can only add an image to the file if it's in PDF format.
