how to add email address to digital signature without distinguished name

When I create a digital signature it lets me input the email address.  The only way I can get the email address on the signature is by changing the appearance and having distinguished name checked.  When I check this it adds my name again, location, and email address.  Is there a way to only have the name, email address, and date?

email is a part of the Distinguished Name and you get either all of it or nothing. You can turn odd Name and/or Date when you create a custom appearance. Try to use the latest Acrobat XI version. I recall there were some slight changes in custom appearances last year (or was it in Acrobat DC only, I don't remember).
