How to CAPTURE video through the PDF

I think this may be a unique request, but I am looking for a method whereby I can include a video record button within a PDF file.  So this works like this, the user of the PDF file would complete any necessary field entries and when completed would be presented with a video record button.  This button when pressed would then allow the user to record and imbed a video specific to themselves, perhaps to document the fact that they have completed the entire PDF.  I want to use this in lieue of electronic signature.

Any thoughts?
