How to correct my PDF Pro from printing a blank form??

I am using eCopy PDF Pro Office 6.1  I've created fillable forms in it before and was able to print them with all of their content; however, now they are printing completely blank- and only the content that is typed into the fields print.  Usually I create the form template in Word, save it as a PDF, and then in PDF Pro, I use the "FormTyper" option in "forms"- it automatically creates the form- I make any adjustments (like deleting text boxes where I didn't need one, making a text box bigger, etc.), and then I'm able to either print the blank form (printing only the background information), or I'm able to type in the fields and then print the filled out form.  Like I said before, now nothing prints except anything that's written into a field.  Please, any help would be GREAT!!!

In the Print dialog, under the "Comments & Forms" option, make sure that "Document" is selected, not "Form fields only".

I am trying to print a license pocket size from another co & has a cover sheet and will not print either one.  Any suggestions I'm a very beginner in this pdf areal
