Creating dynamic stamps is a complex subject. You'll need to spend some time learning it, including how to write scripts to prompt the user for this information and then populate it into the stamp. Try searching the forums. There have been many posts about this subject, including links to tutorials. There's even a whole book written about it by Thom Parker, who frequently posts here.
