How to create a form field that will automatically generate another one below it with same rules.

I think I did not explain my question properly! Forgive me.

I am looking to be able to create a form text field that when clicked on and information is inputted, another form field will form below it. The user then if continuing to click on the new form fields will keep generating a text box to fill with information until they do not need any more entries.

I want to be able to have my form user be able to print out a customized form with the exact number of fields needed without me having to format the form with a certain number of fields only that will unnecessarily print.

The form must adhere to specific formats so a lot of blank spaces is not good!

Copy the fields from the first page and then paste them in the second one to get identical copies. Fields with the same name will automatically have the same value, so if the user edits them on one page it will update the rest of them as well.
