How to create a "Tax" Box that calculates taxes according to "Subtotal" Box

Hello, I am using Acrobat Pro 11. I am trying to create a "Tax" Box that calculates taxes according to "Subtotal" Box. Can anyone please help? Thank you in advance

Let's say the tax rate is 11%. Under the Calculation tab of the tax field select the second option ("Simple Field Notation") and enter this as the calculation formula:

Subtotal * 0.11
