How to load fields from selection in drop down menu?

Basically I have 10 inspection sheets at work we call "take-off's". Each take-off is specific to a type of inspection.

Instead of making 10 fillable PDF forms of each take-off, I wonder if I can have a "Master Take-off" PDF. Within the document there will a drop down menu with options to select such as:

Man Door take-off

Dock Seals & shelters take-off

Overhead door take-off

and so on....

Upon selecting one of these in the drop down, in the same PDF form, it will only load/display the fields applicable to that selection.

I think this is possible, but am having a hard time for whatever reason researching this topic and finding the solution.

Can anyone help provide a tutorial or even a template I can work off of?
