I am having trouble with basic text field forms

I have a PDF document which I created in Photoshop.  It is a pretty standard form that clients of mine fill in.  I saved it as photoshop PDF and then opened it up in Acrobat.

I have used field forms before to put in text fields and the like, however I am running into a problem for some reason on this document.

When I create a new text field form and click on Properties to edit, it is only showing 4 tabs in the Text Field Properties box.  I am only getting "General" - "Appearance" - "Position" - "Options"

I am missing the other tabs for "Calculate" and "Validate".  Does anyone know why these tabs aren't showing up?

I have tried opening other PDF documents in acrobat and they all do show up, so could there be a problem with my PDF?
