Issue in creating fillable form

Why am I getting the hour glass when I attempt to make a PDF form fillable with Acrobat XI Pro?

Does anyone have any suggestions for my software not populating the fillable fields in Acrobat XI Pro? I'm just getting the circle (like the hour glass...working, but not really).

Could you please confirm the dot version of Adobe Acrobat XI you are using

Also let us know the OS installed on your system.

Please share the exact steps you do fill form and screenshot of the page you get.
