Link from table of content to corresponding page

Following problem:
I have a PDF with a table of contents on page 1.
Can I link every point in the table of contents to the corresponding page?
If yes, how do I do that?

As mentioned, you can do it manually using the Link tool. However, it's very likely to also be possible to do it automatically, using a script. You can use the link tool to add links between any two pages - one at a time.

But a TOC is normally made in the original app, like Word or InDesign, and exported complete and working with links.
