Need help with custom calculation in form

I have a custom form built in PDF and I need the user to be able to type in their Dollar amount at the top ( $10,000) and then have the corresponding breakdowns for each month - i.e. 5.5% of $10,000 in January, 6% of $10,000 in February.

Is this possible? I am afraid this is way out of my scope!
