From an earlier reply it sounds like you (perhaps inadvertently) set up the document to be an Adobe Sign compatible form, which happens when you select the "This document requires signatures" check box when you select Tools > Prepare form initially. Adobe Sign forms normally wouldn't allow buttons, but if you add one while not in form editing mode it won't then let you specify an action. Anyway, the fix is to go back into form editing mode and select "More > Revert to Acrobat Form", and any buttons you add will have the Actions tab available.

You don't have to be in form editing mode, and can instead use the button tool that's available in the Tools > Rich Media panel, so it's also available to be added to the Quick Tools and a custom toolset.

Thanks, George! You got me one step closer! However, there doesn't seem to be an actions tab for that button. How can I assign this button to clear all form fields? (this used to be so easy before DC!)  Thank you again for your help!

You have to be in the Prepare Form tool.
