PDF Form problem - Data from 1st page populating next 3 pages

I have a PDF form, four pages.  When we fill in the first page of data, the information automatically goes to the next three pages.  So, we have to fill a page.  print.  Fill 2nd page.  print. rinse and repeat.  Could it be the text box naming causing the problem?  The problem is the 2-4th pages are very different data but the first page populates them anyway.

Any help would be greatly appreciated; please note that I inherited this form, am not very knowledgeable about PDF forms (yet) and really need to figure out what might be happening.

Note that in order to change the field names, you'll have to have access to Acrobat, or perhaps LiveCycle Designer, depending on the type of form it is. If it is currently secured to prevent changes, you'll need to know the security password in order to remove the security and edit the fields.

Give each field a unique name.
