PDF Form & Signatures Problem

Howdy,

I have created an enrolment form in Acrobat Pro DC which we want staff to be able to partially complete and send via Adobe Sign & the client to complete and sign.

However, I'm having issues with clients missing fields and the document being locked after being signed OR the client never received the form and staff are having to retype all the info in and resend.

Any ideas?
