PDF Form with multiple pages auto copies text into fields on next pages

I created a fillable PDF form with multiple pages. I started out with an Illustrator file, then exported it to Acrobat to make it fillable. For some reason, though, when users enter text on the first page and try to continue onto the next, the text from the first page just auto copies over onto the following pages. And if you delete what was on the second (or third) page to enter the correct data, it will populate that on the first page.

I figure it must be something that went screwy when I was creating the form, so I don't know if there is a way to fix it without just starting over.

Give each field a unique name. Fields with the same name are copies of each other and will automatically have the same value.
