PDF survey tool with "bubbles" and words typed sideways! How to convert to Word?

I need to export a PDF into a Word document but the PDF is a survey tool with "bubbles" and some words printed sideways. Can anyone offer a suggestion about how to convert such a PDF to Word? I have Adobe Export PDF but will upgrade if necessary.

The thing is, Word is a tool for simple layouts. If Word could do complicated layouts like this, PDF would never have been invented! So converting to Word should not have any expectation of "it will be exact, no matter how complicated". Rather, it is a starting point for when the original is lost and there is no other hope of recovery.

What are the results when you convert it as is? There's probably not much that can be done about it, though, except for editing the file, either in Acrobat or in Word.

Thank you for your reply. When I converted from Adobe to Word, the bubbles became various symbols and the words that appeared sideways in Adobe (above the bubbles) were symbols. I ended up typing the whole thing in order to meet a deadline. I thought there might have been some kind or OCR program available but never found one. Thanks, again.
