PDFs exporting Blank

Hi, I have made a PDF into a form that I am able to edit and fill out daily as notes for my job. I would like to send these notes to other people. However, when I send as a PDF it opens as blank with my notes not added. I've tried exported as an image and sometimes this works and other times it opens without notes as well. Help!!!
