Possible to create form with this requirements?

Hello

My question is, is it possible to create a form with the following requirements?

1. Open PDF

2. The supervisor fills in fields.

3. The manager clicks on one of the departments, then the form should be forwarded to defined, stored e-mail addresses for processing (6 departments, on each departement is an email address - the choice of the departements with radio button or dropdown field).

4. Example, the email was send to the department secretariat. He write an opinion in a field (including the name of the editor and the timestamp) and forwards it to the personal office by clicking on «Continue» (two defined, stored e-mail addresses, the choice of the personal office with radio button or dropdown field).

5. The personal office also write an opinion in the form (including the name of the editor and the timestamp) and forwarded it to the department (send back -Department that is mentioned in step 2.).

6. The department prints out the form.

is that possible to create with adobe and javascript??

Yes, this is possible. And there are a few different way it could work. For example the easy way to so just create Forward buttons for each individual in the process. But it could also be programmable so the process only moves forward. The more complex, the more complex the scripting. Here's are two articles that show how the emails could be programmed
