Reader Enabling my Dynamic PDF isn't working?

I have a dynamic pdf with cells that expand vertically with text. When I tried to reader enable it through Acrobat XI Pro, it adds additional repeat rows to my original table.

It should look like "Worksheet 15A Sample.pdf", but instead I get "Worksheet 15A Sample Reader Enabled.pdf". Reader enabled document does eventually stop repeating and continues with the original content. Any ideas what happened? Any ideas on how I can fix this?

The dynamic forms were created on LiveCycle Designer ES4, if that makes a difference.
