Sending filled PDF to email from a WordPress site

Hi all,

we have a minor problem with one of our clients. They want us to make a PDF (a registration contract) that their clients can fill in the website and then send it to our client by e-mail. Is there a way of doing that? We are doing websites with WordPress. Thanks a lot for your answers!
